Dear Parent or Guardian,
Wisconsin has updated its immunization requirements for child care and school entry for the 2024-2025 school year. Immunizations are one of the most important ways to protect children from diseases. Immunizations are especially important for school-aged children because children in school are regularly in close contact with others who may be able to spread these diseases. Changes include:
- Adding a meningococcal (MenACWY-containing) vaccine to the requirements for students entering 7th grade, and a booster dose for eligible students entering 12th grade.
- Parent report of chickenpox disease is no longer acceptable for exemption from the varicella (chickenpox) vaccine requirement. Children must have a diagnosis of chickenpox from a qualified health care provider (M.D., D.O., N.P., or P.A.) to be exempt from this requirement, however existing exemption options still apply.
For the upcoming school year, all students will need the following immunizations or an appropriate health, religious, or personal waiver.
Kindergarten - 6th grade:
- 4 doses of Polio
- 4 doses of DtaP/DT/Td
- 3 doses of Hepatitis B
- 2 doses of Varicella (chickenpox) or history of disease documented by a qualified heath care professional
- 2 doses of MMR (measles, mumps, rubella)
7th-11th grade:
- All the previously required vaccines, plus:
- 1 dose of Tdap
- 1 MenACWY-containing vaccine

- Make a vaccine appointment with your doctor or the Kewaunee County Public Health Department. The Kewaunee County Public Health Department is offering a FREE vaccination clinic on the following dates:
- Wednesday August 28th, 2024: Kewaunee County Expo Center 625 Third Street Luxemburg Time: 2:30pm – 5:00pm
- Wednesday October 9th, 2024: Kewaunee County Health Department 810 Lincoln Street, Kewaunee Time: 2:00pm – 5:00pm
- Please contact the Kewaunee County Health Department at 920-388-7160 with any questions.
- Ask the health care provider to complete the Student Immunization Record form (see attached). Return the form to your child’s school.
- You can also visit the Wisconsin Immunization Registry to see if your child is missing or coming due for any immunizations.
